Orthopedic Surgery Appointment Request Form

To request an appointment, please complete the Appointment Request form below. To make an appointment as rewarding as possible, we strongly encourage you to provide us with all the information requested. This includes:

• Filling out this questionnaire to the best of your knowledge.
• Sending us X-rays (radiographs) and veterinary records that are related to the current problem. If you do not already have this information, you can request it from your veterinarian and email us later, but please do this as soon as possible prior to your appointment (which will allow us the chance to review the information provided). Please email this information to ortho@colostate.edu. Please include your pet’s name and your last name in the file name of anything you email.
