It is past the deadline to submit the course evaluation request form for the 2026-27 application cycle. If you submit a form, you may not receive a response by the Sept. 15 application deadline. This also includes separate email submissions.
To submit a request for a course substitution for one of CSU’s veterinary prerequisite requirements, please complete this form (must be submitted by September 1).
REQUIRED: If your course evaluation request is approved, please provide a one-sentence explanation to explain the course approval in the “Special Circumstances” section of the Colorado Supplemental Application.
