Small Animal General Surgery Appointment Request Form

To request an appointment, please complete the Appointment Request form below.

In addition to filling out the Appointment Request form, please provide veterinary records related to the current problem, including:

• Blood work
• X-rays
• Reports from any advanced imaging including ultrasound, CT scan, and/or MRI

If you do not already have this information, you can request it from your veterinarian.

Please email your pet’s veterinary records to softtissuesurgery@colostate.edu as soon as possible prior to your appointment to allow us the chance to review the information provided. Include your pet’s name and your last name in the file name of anything you email.
